What we collect, why we need it, and who helps us handle it.
This policy explains how Advanced Vision Care Center LLC (“AVCC”) handles information collected through advancedvisioncarecenter.com. The website helps you request an appointment or ask a question. It is separate from our Patient Portal and in-office intake process.
Our Notice of Privacy Practices covers health information across the practice, including protected health information in a request for care. Those protections apply before you complete patient intake, too.
What you share
- The appointment request asks for your name, phone number, email, preferred office, general reason for visiting, scheduling preferences and payment or insurance-plan name.
- The question form asks for your question, name, phone number and email. Choosing an office is optional.
We use your email to send your appointment confirmation after the office confirms the details with you by phone.
Please leave health details, insurance member IDs, dates of birth and Social Security numbers out of both forms. We don’t ask for them here. If you include health information, we protect and handle it under our Notice of Privacy Practices. Ask the office how to share medical information securely.
How we use your booking request
We use information you submit through the website to contact you, answer your question and arrange your appointment—not to provide treatment or process payment. We don’t sell personal information or use your request for third-party marketing.
Your medical history, examination, treatment and billing are handled separately through the practice’s intake and care processes, including in-office forms and the Patient Portal. The website forms do not replace patient intake.
Who handles it
Your request goes to the appropriate AVCC office and doctor. Our team contacts you to confirm the details and arrange a time. Any information staff enter into the patient system is handled as part of the practice’s process; the website has no direct connection to that system or the Patient Portal.
Service providers help run the website and deliver requests. We may also retain or disclose information to meet legal obligations, subject to applicable privacy protections.
What happens when you browse
- We don’t use analytics, advertising pixels or tracking cookies.
- Your browser stores a submission timestamp to help prevent repeated form requests. It doesn’t contain your message or contact details.
- Our website host may process technical details such as your IP address, browser information, pages requested, request times and response status to deliver and protect the site.
- The site also loads fonts from Google, which receives technical information when your browser requests those files.
Keeping information safe
The website uses encrypted connections yet no internet transmission is completely secure. Please keep medical details out of website forms and ordinary email.
How long we keep it
We keep request information as needed to respond, arrange your appointment and meet legal obligations. Information that becomes part of your patient record follows our medical-record retention policies.
You can ask about retention or request deletion. Some information must be kept by law or for other permitted purposes.
Your choices
- Call an office instead of using a form.
- Tell us how you prefer to be contacted.
- Ask us to correct information you sent.
For parents and guardians
These forms are intended for adults, including parents and guardians requesting care for a child. A parent or legal guardian is generally expected to accompany patients under 18. Ask the office about exceptions or consent arrangements.
